Free Community Tennis Day (Juniors)

1. Assumption of Risk:
Assumption of Risk: I, the undersigned, acknowledge and understand that participation in tennis events involves inherent risks, including the possibility of serious injury, death, or property damage. I am voluntarily allowing my child (or the child I am responsible for) to participate in the tennis activities, and I understand the risks involved, including but not limited to:

Injuries due to slips, falls, or collisions

Muscle strains or sprains

Broken bones or joint injuries

Head, neck, or back injuries

Injuries from tennis equipment (racquets, balls, etc.)

2. Medical Treatment Consent:
Medical Treatment Consent: In case of an emergency, I authorize the event organizers and/or staff to provide or arrange for emergency medical treatment for my child (or the child I am responsible for) if necessary. I understand that the event organizers will make every reasonable effort to contact me or another designated emergency contact.

3. Waiver and Release of Liability:
Waiver and Release of Liability: In consideration of being allowed to participate in the tournament, I, on behalf of myself and my child, hereby release and hold harmless the event organizers, sponsors, staff, volunteers, and all individuals or entities connected with the tournament from any and all claims, liabilities, damages, or expenses arising from or related to my child’s participation in this event.

This waiver applies to all injuries, damages, or losses, whether caused by negligence, recklessness, or otherwise.

4. Parent/Guardian Consent:
Parent/Guardian Consent: As the parent or legal guardian of the participant named above, I grant permission for them to participate in the tennis tournament. I affirm that my child is physically fit and capable of participating in the event. I also consent to the use of my child’s image, name, or video for promotional and marketing purposes, including event photography, video footage, or social media.

5. Acknowledgment of Rules and Regulations:
Acknowledgment of Rules and Regulations: I agree to abide by all tournament rules and regulations as set forth by the event organisers. I understand that failure to follow these rules may result in disqualification from the tournament. I also understand that the decision of the event organisers and officials is final.
